Instructions for Use:

Process Overview: All credits and refunds are now managed centrally and must be requested through this form. Store-level staff including management are not authorized to process credits directly.
Resolution Methods:
- For credit card payments: Credits will be applied to the same credit card used for the original transaction.
- For cash payments: A gift card for the amount owed will be mailed to you.
- For missing items: We will kindly deliver any missing item to you. In such cases, please contact the store directly to arrange the delivery.
Assistance: If you are unable to fill out the form yourself, one of our employees will gladly assist you and complete the form on your behalf.
Response Time: After submitting the form, our team will review your request and contact you within 3-5 business days with a resolution.
